My name is Mr. Chris from Parañaque City. My mother obtained her
PWD ID card from the city social welfare office. Her application was
kindly facilitated,and coordinated by the very helpful staff members /
employees of our Barangay Moonwalk office, to the concerned offices
of our city hall. My mother has chronic illness, cardio-vascular disease.
She is being granted significant discount on her fee for her regular
physical therapy by the private hospital where her PT session is being
conducted, and on her purchase of prescription / maintenance medicine.
She can also avail discounts in other products / commodities, and
services itemized in the booklet issued along with her PWD ID card.
I hope those below 60 yrs of age who are handicap / physically challenged /
differently abled / with other disability (ies) as defined / categorized by
pertinent offices / agencies should apply for Persons With Disability ID card,
so that they may also enjoy the same benefits / privileges entitled to them.
Thank you to those who share to me the information I mentioned, and
to those who are instrumental for my mother to acquire her PWD ID card.

y is it plaridel bulacan was not listed among municipalities issueing PWD IDs ? were done, using PSGC in numbering their IDs.PRES.AYE PLARIDEL CHAPTER

how’s about not find the location in zamboanga city? bec they are special deafs and disibility person many the dont have apply ID about need discount for help any problem give important there in zamboanga city. but wer find the get wala pwd id or ncda id it can reason why.

Currently employed as Focal Person on PWDs from San Juan, Batangas. We are also issuing IDs and PDAO office is already under construction

bakit hindi ho naisasama ang bayan ng Marilao, Bulacan sa nakapag-issue ng mga PWD’s I.D. Bilang pangulo ng samahan ng mga may kapansanan sa Marilao patutunayan ko ho na may mga I.D na ho ang marami naming kasapi.

Ako po ay disabled at cguro nman po ay entitled ako sa mga benefits at discounts na pwde ko ma-enjoy sa PWD IDs…pero ang problema ko po eh bkit po wala sa listahan ang Municipality ng Tanay???Akala po ba nila eh walang PWD sa Tanay???

In Region III Province of Bulacan Municipality of Plaridel, since last November 2009, we’ve been issuing PWD ID’s.

May be, the NCDA can coordinate with DSWD focal persons for PWDs for updates re: list of LGUs issuing PWD IDs in Region 01. Region 01 has more than the number reflected above.

we would like to inform also that since 2008 we are already distributing PWD IDs and purchase booklets, as well, to our differently abled constituents of Bangui Ilocos NOrte. I am sharing this because i have read in your report that you only mentioned Laoag City.

My son is already 24 yo. He is mentally retarded (Severe). He was issued of his ID since 2009 here at our municipality (Bay, Laguna). I was availing of a Discount Booklet to be used in the purchase of his medicine for his maintenance, kasi ang mahal ng gamot nya. I’ve requested for this booklet since last year but up to this moment WALA pa rin silang maibigay. How long shall I wait for this booklet? Pwede bang paki TAP yong office ng DSW sa aming municipality.

my father has glaucoma but he is in very good health. he has worked abroad as a clerk for many years. before his glaucoma worsened he always plays chess and received awards for playing. right now, we’re living in davao city. i would like to know if there’s an office for persons with disabilities here in davao city.
my father still wants to be productive be it in chess or whatever career like medical transcription for the blind. thank you and God bless!

I would like to ask if my child who’s 7 yrs old and diagnosed with ADHD w/ conduct disorder can qualify for a PWD ID? And we live in Angeles City but i saw from the list that it does not issue such here. I saw that City of san Fernando issues these IDs, can we apply for that there? I am a single parent and his medications are really expensive plus the therapy sessions he needs to undergo. Do the discount scheme cover these? And by the way how do i go about applying for the ID again? Your assistance will be very much appreciated! Thanks very much!

I am a widowed mother of a pwd, i have applied an id for my daughter and have enjoyed the privileges. i have been asking municipality of san pedro for grocery purchase booklet so we can avail of discount on grocery. its taking already years before the mnunicipality release the grocery purchase booklet, they say it is still subject to printing and approval. HOw long does this process take? who approves it? is it local ornational govt?
How can i go to the process of submitting information with the Internal revenue for the exemption of my dependent. she is already 21 yrs old.
please advise.
