1. Fill out the Philippine Registry Form and submit the documentary requirements
2. Philippine Registration Form for Persons with Disabilities (PRFPWD) shall be verified/processed
3. ID shall be issued once processed

Note:

For all those seeking the ID for Persons with Disability (PWD) as proof of entitlement of the 20% discount please go to your Municipal/City Social Welfare and Development Offices (M/CSWDOs) which are usually located in Municipal/City Halls in your area for the ID issuance. NCDA only issues IDs to soldiers with disabilities.
