Here is the list of Cities and Municipalities which are issuing the new Persons With Disabilities (PWD) Identification Cards as of August 14, 2009.

      I am not a PWD but there are many PWDs here in Lopez, Quezon, especially children but where can they go to register and/or apply for assistance.

      magandang araw po sa inyo, asawa ko po ay isang disalble at tatay ko rin po, paano po kami maka hingi ng tulong sa mentainance ng gamot sa tatay ko. Sa asawa ko naman po polyo po sya.Sana po kamin ay inyong matulungan at maraming salamat po.

      Good day po!

      My sakit po ang brother ko 22y/o diagnosis was Schizophremnia, his medicines are so expensive and we wish to apply for PWD ID were we could at least have discounts in purchasing medicines.I am from Municipality of Carranglan Nueva Ecija and asking for guidelines in requesting PWD ID. TYVM.

      To get an ID you need the following:

      Medical Abstract/Certificate –
      Barangay Certificate – this is just to certify the residence of the PWD.
      2X2 ID picture
      Signature/Thumb mark – you can do this in advance and have your child’s thumbmark on a clean piece of paper. They will use the thumbmark in the ID as the signature.

      I am a person with an orthopedic disabiity. I joined and was voted as a PWD President in one of the municipalities here in Region 8 last December 10, 2015. I am aware that the term for rendering service of a PWD Official is three years. I was 59 years old then when I joined the organization in 2015 and I have only rendered one year service as a PWD President. I turned 60 years old this December 6, 2016. Do I still have the right to continue to render service until December 6, 2018?

      I went to the office of PWDs in Molo, Iloilo City, Iloilo City last Tues for the renewal of my PWD ID that was originally issued to me in Miagao in 2015. The staff at I can do so because while my home address is at Ungka I, Pavia, it is near Iloilo City. She told me to secure a Barangay Clearance/Certificate and return to their office.

      To my dismay this morning, another set of staff told me that their office cannot accommodate my request because I am not from Iloilo City. I asked them where could I seek assistance nearby. I can ask the DSWD and Office of the Mayor to approve my renewal even if I am no longer a resident there but Miagao is 45 kilometers away!

      PWD IDs can be issued by any mayor and honored anywhere in the country. Why can’t your agency make a policy that any Filipino citizen apply anywhere in the country? I will write the Bahay Ugnayan in Malacanang about this.
